25/4/15 He was at the Gallipoli landing
20/8/15 He was admitted to the field ambulance at Anzac with diarrhea
22/8/15 He was moved to Mudros and admitted to the 18th Stationary Hospital with enteritis
19/9/15 He was admitted to a convalescent camp at Mudros and discharged
25/9/15 He went back to hospital on Lemnos

He was in action at the Somme
25/7/16 He went to the 44th Casually Clearing Station where he died
His effects consisted of an identity disc, leather money belt, jug purse, whistle, lanyard, pocket knife and chain, stud, notebook, post cards, letters, photos and a battalion colour

3/5/17 His death certificate was sent to Mr. J W Sutherland solicitor of Leongatha. Mr. Sutherland requested this, as it was required by an insurance company.

His mother was married to a Lockwood, the soldier�s father died in 1896
Her other sons were J G Phillips 3rd DAC and Captain A F Phillips 7th Battalion AIF
